What to expect when you enroll

1. The parent should enroll themselves as a patient, but only fill out the demographics, not medical information.  This is necessary to be able to group the child(ren) under the parent, and allow the parent to have access to the Patient Hub. The Patient Hub allows for scheduling appointments, communication with the doctor and staff, refill requests, and to make changes to billing options.  There is no charge to the parent, and they are not considered an actual patient of the practice.

2. Next enroll the child(ren). Fill out as much information as you can. A welcome message with additional instructions will then be sent to the parent. Once the child is accepted, the parent will get an invitation to go onto the Patient Hub and enter their preferred payment method. This needs to be done before enrollment is considered to be complete.

Payment methods include HFA, Credit Card, Bank Transfer, Check, or Cash. Let us know if you want to pay via Check or Cash. Additional discounts are available for these methods.

3. Please be certain to read the enrollment agreement before you agree to the Terms!  To access this, you must click on the link. Updates to the agreement will be made available via email upon request.
